FOGO trial survey Terms and Conditions

• To enter, entrants must complete the survey on the Collaborate Canada Bay page at bit.ly/fogotrialsurvey and fill out the required fields. The survey commences on Monday, 6 November 2023 at 9am AEST and closes 11:59pm on Sunday, 30 June 2024.
• By completing the survey, all respondents will be deemed to have accepted and agreed to be bound by these rules.
• All respondents must be over the age of 18 years or, if younger, responses must be made with consent of a guardian or adult over 18 years old. Survey must be completed by participants of the FOGO trial and addresses provided will be cross checked by employees of the City of Canada Bay.
• Employees of the City of Canada Bay and agencies associated with this promotion can complete the survey, however, are ineligible to receive the e-gift card, as are their immediate families.
• The prize is one (1) e-gift card totalling a value of twenty ($20) dollars AUD. There is a maximum of 100 e-gift cards and only one (1) per eligible household to the first 100 respondents. The prize is non-transferable and non-redeemable for cash. E-gift cards will be sent out to the email addresses provided by the respondents.
• The information you are asked to provide to the City of Canada Bay includes personal information and is protected by the Council’s Privacy Policy. The City of Canada Bay will not disclose personal information to other parties except as permitted by law, or subject to clause 21. • If respondents do not respond to Council’s email notifying that they are the winner within 30 days, Canada Bay Council may award the gift card to another valid entrant.
